Year 8 HPV Letter (a form needs completing to give or not give consent)

Polite reminder that the school vaccination team will be in school on Tuesday 1st July 2025 to administer the HPV vaccinations to those who didn't receive the vaccination on 13.02.2025.
If you have not already given your consent, this can be done on the following link using the unique school code: WF139335
If you wish your child to receive the vaccination, please complete the ‘I do consent’ section.
If you do not wish your child to receive the vaccination, please complete the ‘I do not consent’ section.
No pupils will be vaccinated without express permission from parents. For more information follow https://www.nhs.uk/vaccinations/hpv-vaccine/ or call the Immunisation Team on 01924 310133 and they will be happy to advise.
The immunisation team need consent in by Tuesday 24 June 2025.
