Covid vaccination consent forms.

Dear Parents/Carers,

Today, Monday 4 October, students aged between 12 and 15 have been given a Covid vaccination consent form to take home. All completed consent forms are to be returned tomorrow Tuesday 05.10.2021 to SID or on Wednesday 06.10.2021 at the very latest. We will provide more details shortly of when the vaccinations will take place at a later date. 
**Any consent form that is returned after Wednesday 06.10.2021 will result in the child not receiving the vaccination.**
